Trinity Pathway Hospice 

Trinity Pathway Hospice is a program of care for terminally ill patients and their families. The patient must have a limited life expectancy and understand that curative treatment is not provided. Hospice is a Medicare Certified program, which means that under this benefit, patients receive all services, medications, supplies and durable medical equipment related to their terminal illness without co-pay. Bereavement follow-up is provided for families.

Trinity Pathway Hospice is offered in two formats: a home care program and in-patient services. In the former, a medical director, nurses, home health aides, social workers, chaplain, dietitians, PT, OT, ST and bereavement counselors work with terminally ill patients and their families under the direction of the patient's attending physician. Hospice provides symptom management and addresses end-of-life issues patients and their loved ones face. Respite services are available when caregivers need short term relief.

At the Trinity Pathway Hospice facility, patients and families are provided a homelike setting that ensures safety, comfort, and caring for the patient, as well as support for the patient’s family as they prepare for their loved one’s death.

Here, an interdisciplinary team that includes a medical director, nurses, certified nursing assistants, social workers, chaplains, physicians, therapists, volunteers, and most importantly, the family, helps customize the patient’s care plan.


Inpatient Unit Features

  • Patient Rooms:Patient rooms include homelike décor versus institutional furniture. Each room also has a sleeper chair for a family member to stay overnight. Special beds have gel form mattresses that prevent skin breakdown. All beds also include cozy bedspreads and sheets instead of traditional hospital linens. Other touches include softer lighting, table lamps and dressers with increased storage, artwork on the walls to hide medical equipment hook-ups as well as a DVD/VCR player.
  • Kitchen Area (available for use 24/7): Family members often cook meals together in a hospice setting. The fully-equipped kitchen includes a full-size refrigerator (partially stocked with some essentials), stove, microwave and dishwasher as well as dishes, pots, pans and utensils.
  • Family/Dining Room: Includes eight-person dining table and chairs, 190-gallon salt water aquarium and individual seating areas with chairs, couches, floor lamps, table lamps and end tables.
  • Laundry Room: Family members are able to do their personal laundry without ever leaving the unit in a laundry room that features a washer, dryer, full-size refrigerator, folding counter, table with chairs, and laundry detergent.
  • Kids Town: Because there are no age limits placed on those visiting the facility, a playroom decorated in bold, primary colors has been created for young visitors. It includes a television, games, toy bins, play tables and a chalkboard.
  • Reflection Room: The reflection room provides a serene area for families to simply clear their minds or access grief support books and other literature. The tranquil room contains a trickling fountain, dim lighting, and a journal in which family members or other visitors can express their feelings through written words.
  • Salon Area: If a family member wishes to have a stylist come into the facility for their loved one, they can make use of the salon area complete with shampoo sink, counter tops and cabinets.

Care Levels Tailored to Your Needs

Under hospice care, the patient must have a limited life expectancy of six months or less, and the patient chooses not to have curative treatment. Hospice provides symptom management and addresses end of life issues patients and their loved ones face. Bereavement follow-up is provided for all families.

Trinity Pathway Hospice offers three levels of hospice care:

  • Short term respite care - For current Iowa home care hospice patients, respite care is available for up to five days in the residential facility; 100 percent coverage by Medicare or most other insurances for room and board as well as all services, medications, supplies and durable medical equipment related to their terminal illness without co-pay.
  • Acute care – This option is for hospice patients being cared for at home who experience a medical situation requiring 24-hour monitoring by nursing staff. The patient will be brought to the facility to help get the symptoms under control before returning home; 100 percent coverage by Medicare or most other insurances for room and board as well as all services, medications, supplies and durable medical equipment related to their terminal illness without co-pay.
  • Routine care – Residential care for patients with limited life expectancy less than six months. Private pay room and board; Medicare or most other insurances for hospice services providing symptom management, including medications, supplies and durable medical equipment related to their terminal illness.
